In this role, you'll play a pivotal part in providing top-notch social case work and crafting insightful social histories for our clients and their families. Your expertise will help patients and their loved ones navigate their health journeys, ensuring they receive the support and resources needed for optimal well-being. What You’ll Do:

• Conduct psychosocial assessments and develop actionable plans with precise referrals.
• Offer rehabilitative and supportive casework to help patients achieve the best possible social and health outcomes.
• Maintain a high standard of professionalism and confidentiality, adhering to HIPAA guidelines.
• Collaborate with community agencies and coordinate referrals to ensure comprehensive support for our clients.
• Engage in ongoing education to stay ahead of the curve in the field of social work.

Education & Credentials:

• Master’s Degree in Social Work from an accredited institution
• Current and unrestricted licensure to practice as a Medical Social Worker in the state
• Required health and screening tests as per facility requirements
