Job Description

Menorah Park of CNY seeks an experienced Social Worker with a background in clinical psychosocial responsibilities. The ideal candidate should be comfortable helping patients adapt to their limitations and assisting families with important decisions about their loved ones.

• Provide individual counseling and supportive consultations
• Experience effectively managing referral and case management practices to ensure social-emotional and social service support
• Engage residents and/or families in professional treatment relationships to assess the need for Social Work intervention.
• Assist departmental staff in the facility on matters related to Social Services policies and procedures and overall resident care policies.
• Act as liaison to the handling and resolving of resident grievances and concerns.

REQUIREMENTS:

• 2+ years clinical social work preferred
• Bachelor's Degree is Social Work required
• Ability to make psychosocial assessments and develop and implement viable care plans
• Excellent listening and communication skills
• Experience with EMRs
• Familiar with DOH guidelines preferred
• Ability to provide after-hours service as needed
• Experience in a LTC/SNF preferred
